Sunflower Assessments LLC appreciates you choosing us for your child's care. To maintain a focus on your child's well-being and minimize the administrative costs associated with contracting with insurance companies, we operate as an out-of-network provider.
Payment Policy Statement
Payment is due in full at the time the appointment is confirmed for scheduling.
• No Insurance Accepted: We do not accept or bill any commercial insurance, Medicare, or Medicaid plans directly.
• Out-of-Network Benefits: If you have health insurance with out-of-network benefits (such as a PPO or POS plan), your insurance company may reimburse you for a portion of the services we provide, however reimbursement is not guaranteed and it is possible that this will be an "out of pocket" expense.
• Itemized Receipt (Superbill): We will provide you with a detailed, itemized receipt (often called a Superbill) that contains all the necessary codes and information you need to submit a claim directly to your insurance company for potential reimbursement.
• Patient Responsibility: Submitting this claim is the patient's sole responsibility. We encourage you to contact your insurance provider prior to your appointment to verify your benefits and understand what you may be reimbursed.
• Unfortunately, we have to cancel appointments for which payment is not received.
Accepted forms of payment: Zelle and online credit card payment
The Autism Diagnostic Observation Schedule (ADOS) is a standardized diagnostic tool used to assess and diagnose autism spectrum disorders across age, developmental level, and language skills. At Sunflower Assessments LLC, we employ the ADOS as a core component of our diagnostic process. The ADOS consists of a series of structured and semi-structured tasks that provide opportunities for social interaction and communication. We observe and evaluate these interactions to identify key indicators of autism. By incorporating the ADOS into our assessments, Sunflower Assessments LLC is able to offer precise and reliable diagnoses, helping to create personalized intervention plans that support your child's development and well-being.
We offer a range of assessments including:
- Autism testing (including the ADOS)
- ADHD evaluations
- Evaluations for developmental delays
Families receive a comprehensive report of all findings and a diagnosis letter if applicable.
We do not provide:
- Primary care services (e.g., routine pediatric visits, physicals, vaccinations)
- Emergency care services (call 911 or visit an emergency room for immediate concerns)
- Specialty care services (e.g., evaluations by developmental pediatricians, neurologists, or psychiatrists)
- Medication prescriptions or management
Our assessments and diagnoses support your child's general and specialty pediatricians in their comprehensive care.
Our assessment process is designed to be thorough and family-friendly. First, you'll schedule an appointment and complete intake forms via our patient portal. We then conduct evaluations at your home or child's school to ensure a comfortable environment. Our assessments include clinical interviews, behavioral observations, and standardized testing. After the evaluation, we provide a detailed report and personalized recommendations to support your child's development.

After diagnosis, we provide personalized recommendations and a support plan tailored to the child's strengths and challenges, that can be started and followed until the child's appointment with their specialized pediatrician: Developmental Pediatrician, Neurologist or Psychiatrist.
